This presentation explores the pathophysiology and management of feline chronic kidney disease-mineral bone disorder (CKD-MBD), focusing on the complex relationship between phosphorus, calcium, and hormones like fibroblas growth factor-23 (FGF-23) and parathyroid hormone.
As kidney function declines, phosphorus retention triggers an increase in FGF-23 and a decrease in calcitriol, eventually leading to renal secondary hyperparathyroidism and potential complications such as soft tissue mineralization or decreased bone density.
Management strategies emphasize early diagnostic assessment using FGF-23 as a predictive marker and implementing nutritional interventions, such as phosphorus-restricted renal diets and the strategic use of phosphate binders to maintain serum phosphorus within target ranges.
This presentation highlights the importance of regular nutritional assessments and monitoring ionized calcium and phosphorus levels to prevent or address issues like hypercalcemia and hyperphosphatemia during management.
